Gout

Gout is a form of inflammatory arthritis caused by the buildup of uric acid crystals in the joints, leading to sudden and severe episodes of pain, swelling, and redness. It most commonly affects the big toe but can also occur in other joints such as the ankles, knees, elbows, wrists, and fingers.

 

Key Characteristics

 

Symptoms:

  • Sudden, intense joint pain, often starting at night

  • Swelling, redness, and warmth in the affected joint

  • Limited range of motion in the joint during flare-ups

  • Lingering discomfort that may last days to weeks after the initial attack subsides

  • Development of tophi, or visible lumps of uric acid crystals under the skin, in chronic cases

 

Causes:

  • Elevated Uric Acid Levels: Excess uric acid in the blood (hyperuricemia) can lead to crystal formation in the joints.

  • Diet: High intake of purine-rich foods, such as red meat, organ meats, and seafood, as well as alcohol and sugary beverages, can raise uric acid levels.

  • Genetics: A family history of gout increases susceptibility.

  • Medical Conditions: Obesity, hypertension, diabetes, and kidney disease are associated with higher risk.

  • Medications: Certain diuretics and other medications can raise uric acid levels.

 

Diagnosis:

  • Typically based on patient history, symptom pattern, and physical examination of the affected joint.

  • Joint fluid analysis may be performed to detect uric acid crystals and confirm diagnosis.

  • Blood tests to measure uric acid levels, along with imaging studies in some cases, may help support diagnosis and rule out other conditions.

 

Treatment:

  • Acute Treatments: NSAIDs, colchicine, or corticosteroids to reduce pain and inflammation during a flare-up.

  • Urate-Lowering Therapy: Medications such as allopurinol or febuxostat to lower uric acid levels and prevent future attacks in individuals with recurrent gout.

  • Lifestyle and Dietary Changes: Reducing intake of purine-rich foods and alcohol, maintaining a healthy weight, and staying well hydrated can help manage uric acid levels.

  • Monitoring: Regular follow-up to track uric acid levels and adjust treatment as needed.

 

Complications:

  • Chronic Gout: Repeated flare-ups can lead to persistent joint damage and reduced mobility.

  • Tophi Formation: Long-term uric acid buildup can result in visible deposits that may damage surrounding tissue.

  • Kidney Stones: Elevated uric acid levels increase the risk of kidney stone formation.

  • Kidney Disease: Chronic hyperuricemia can contribute to progressive kidney dysfunction.

 

Prognosis

With appropriate management, most individuals with gout can effectively control flare-ups and prevent long-term joint damage. Lifestyle modifications combined with urate-lowering therapy, when needed, play a key role in reducing the frequency and severity of attacks. Ongoing monitoring and adherence to treatment help support long-term joint health and overall quality of life.
